California Correctional Health Care Services (CCHCS) is dedicated to compliance with the Americans with Disabilities Act (ADA). If you have a complaint alleging that CCHCS is non-compliant with Web Accessibility Standards, please use the form below to submit a grievance. This form should not be used to escalate grievances about care of an incarcerated individual. Please use the Patient Health Care Inquiry program to report concerns regarding health care.
